Evil Incarnate (Mori everyone with Tombstone Myers)
Pulling it off is hard enough as it is. It's unfairly difficult as survivors will jump into lockers (even if they're the last survivor) or even disconnect solely out of spite to screw you out of the achievement.
Can we please get bots or DC husks so people can't DC out of spite?

Depends on what you classify "hard" for the trophies so I'll mention a few.
Taking one For the Team: Arguably the most grindy trophy in the game and by the time you get this one you'll almost certainly have every other survivor trophy. 250 is stupid and it should either be reduced a metric ton or changed to protection hits in general.
Skilled Huntress: This requires quite a bit of skill with Huntress as well as a massive grind since it's downs and not just hits. Hardest killer trophy to do legit by far, even more so then Evil Incarnate. Could be farmed but will still take a while. Should just be 25 downs and not 100.
Left For Dead: Probably the hardest survivor trophy since they added EGC. Requires a lot of things to go your way to get legit. Highly recommend either doing this with a friend who will purposely die to give you a chance or wait until you get a killer who lets you live when you're the last survivor and hope he lets you do the gens. This trophy should be changed to be the last survivor alive when EGC starts and escape through the exit gates.
Edit: As a tip for Evil Incarnate since I see a lot of people mention it is go to The Game map. I got it first try here when I attempted it. The lockers on that map can be hard to find and it's a lot easier to catch people off guard since they don't know what floor you're on. Play the build normally until you get a group that doesn't gen rush you to hell. Locate the hatch when two are left and slug both while mori'ing the 3rd guy on it. Bring the other guy to the exit gate that has the drop down in the middle. Drop the survivor so he falls while you're still up top and by the time you recover you should be able to get them before they even get to a locker since they will have had to walk around a little. It still requires no DC's and a group that isn't perfect but I think doing it like that makes it a bit easier.

are we all forgetting about the Nurse one that requires you to blink 3 times and then grab someone off of something?
especially after the rework that one is insanely hard.

just play a lot of huntress. Youll get it in no time. for a while i was maining huntress and i got so many. i had well over 50 before they changed the requirements to 20 on xbox.

All adept killer ones. There are lots of useless perks and trying to DOUBLE PIP with them is insanely hard. No way in hell i'm ever doing killer adepts with current requirements.
Maybe change them so only one pip is required to get the achievement?

100 time is an awful lot. As discussed before, we can't make changes to achievements on PS4 if they are at least possible.

The Myers trophy.
Every time I tried and got close, someone DCs or has a key or whatever other nonsense.

I think these are harder than usual right now mainly due to the matchmaker. If you get placed against 4 smart survivors who are above your skill level (or a bunch of inexperienced Grey-Ranked ones far below it), it becomes challenging to generate the points you need for all your emblems. It is definitely at its hardest in the red ranks though, where survivors are usually better and pip requirements are harsher.
However, even under the pip system I don't personally think these feel as challenging as some of the other achievements people are talking about here. Usually it's more about learning how to use that killer's power more effectively to accomplish your goal; the perks are just there to help. A strong power used well will go a long way. Getting killers you're trying to adept up to level 50 also helps, both to get the highest personal perk tiers possible for them and more add-ons to work with.
I have all of the Adept achievements completed, and the only killers that gave me a particularly difficult time (took more than 5 adept attempts to accomplish) were Nurse, Hag, and Plague. Mainly just because it took me forever to get a decent grasp on how to play any of them effectively. With only a few exceptions (Trapper, Doctor, and Hillbilly), I did all the rest of these while in the red ranks. Even killers with absolutely horrid personal perks like Wraith and Huntress weren't really that bad, though I might be biased there because they are two of my favorites.

The one in which every survivor has to escape through the hatch, it's dumb and impossible to do if you aren't playing with friends, and the nurse achievements on console too

It is possible just very hard, I’d call it the hardest achievement if you don’t count SWF to help out with surv achievements

I think deliverance is stupid, so even when I had the perk, I wouldn't use it until I got the achievement done.

As many have mentioned above, it's without a doubt Left Behind for survivor (although some of the generator achievements took me quite a while).
However, since I found Evil Incarnate to be relatively easy to come by as killer and since The Nurse had the enjoyment-nerf, I would say From the Void She Kills is now the hardest achievement.
