The PH random torment bug/just visual glitch

I've seen a few posts in the past and I have personally experienced getting randomly tormented by pyramid head while no where near him or near a trail. I recently saw in a game where around the time he downed a surv/may have started a trail a piece of one randomly appeared right next to me. It spawned below the map but this still may be the issue where people get randomly tormented.
You can see as I walk into the room nothing is there but as I come out from around the gen there is a random piece of the trail just sitting in the doorway.
