Using sole survivor

I play on PS4 (I seen it happen on pc as well) and every time I use sole survivor when more then one player dies or is sacrificed the stacks don’t go up and it still shows 1stack, the perk is always light up while in the past it lights up only while blocking your aura, so I don’t know if it works and when.
