PS4 - Game freezes then randomly disconnects when carrying a survivor

This usually happens during the endgame collapse and has happened several times in the last couple days. Whenever I’m about to hook a survivor near the end of the game, the game freezes and disconnects. I now have a one hour penalty because of this.
This has happened most recently when playing Demogorgon but I noticed it with other killers too. If it matters, my build is Surge, Corrupt, BBQ, and Devour Hope.
