Syringe Consumes Medkit Prematurely

PS4, the syringe will sometimes consume the medkit when you are finished healing even though there are still charges left and you don't hit L1 outside a skill check. It seems to happen 100% of the time with the basic brown medkit with these 3 particular add on set ups
