Xbox One "Near-Death Experience" Achievement Requirements Bugged

The achievement stated in the title requires you to escape while only going down once, 25 times. I've done this at least 50 times just this past month yet the achievement tracker refuses to go up nor does the achievement unlock. I've tried downing for the first time in the match on the exit gate line, downing once without getting hooked, downing once and getting hooked once yet none of that worked. I've also spoken with a few other people that have the same issue so hopefully maybe this could be looked at by a dev. Thank you.
