Is Head On broken on PS4?

I equipped Head On, to do my stun challenge, last night. The seven times I have tried to use it. Only two of those times it has worked.
I thought that the first few times was the killer has pressed the a button before I pressed my prompt. But then I had a nurse who finished checking one locker and just as she got to the other, I pressed my buttons and nothing happened. She got me.
Then I just went against a Huntress who walked right by my locker, I pressed my prompt, nothing happened. She came back to my locker and got me again.
When I say nothing happens, I mean absolutely nothing. Its like the game isn't picking up the command.
