Instant death on 2nd hook

I'll die while mashing on 2nd hook, it's happened 3 times in only about 2 weeks of playing the game. Twice it was instant, once it was with a quarter of the bar left.

Was there a survivor still alive? If not, thats intended since the 3.7.0 midchapter patch.
