Have you followed all the required steps that Deliverance demands?

1: Make a safe unhook (the unhooked target is not downed before the 10 second timer is up).
2: Been hooked for the first time yourself, and attempt escape to use your token and get off for free.

If any luck add-ons are affecting you, the percentage rolls back over from zero, giving you an abysmally low escape chance.
