The swamp maps are a mess.
Two fat-shaming spots on Pale Rose that is a known issue. But it only affects Killer gameplay so that will probably get fixed in 1-2 years.

I don't think the spawn chance is actually 100% - I'm pretty sure I've played games recently without totems in at least one of those spots.
