I'm guessing it makes the game surround sound? I'm not sure since I've always used headphones.

If you have speakers set up and headphones in a headphone jack on your computer, clicking that in the menu silences your speakers and pushes all the sounds to the headphones instead. If you are however the kind of PC gamer who knows how to flip to headphones manually through your default playback Device settings, then this is pretty much useless. What's worse is that if you have a good USB with SSE or Surround sound headphone set, and click that, the game sets your headphones to Default Stereo sound instead of using the surround sound or Surround sound emulation. Which is honestly fine for most people, but for those who have taken the time and fine tuned their surround sound emulation balance settings perfectly for DbD (distance, Bass, ambience, etc)... its a kick in the crotch.

I've never noticed a difference whether it's on or off. And yet I always check/uncheck it if I'm using headphones or not. It would be pretty hilarious if we found out it was a placebo this whole time haha.
