Blood Warden bugged

I play on PS4. When I have Blood Warden active, sometimes survivors are able to escape despite other survivors being clearly unable to. This has happened twice to me on 2 different games over the course of 3 days. Once I first noticed it, I kept an eye on other occasions if survivors escape through my Blood Warden or not. I believe that survivors can sometimes pass through the very left side of the exit gate (when looking into the gate from the entrance) through an unknown glitch or possible unintended opening.
