Safety pipped but depipped?

Just played against a Blight at one of the Coldwin farm maps (I think it was Rotten fields - killer shack was in middle)
and when the game ended my bar progress showed that I safety pipped but rank progress was -1 and I lost a pip.
I was playing on STEAM/PC.
Running Prove Thyself, We are gonna live forever, Iron Will and BT. Playing as a Meg.
I've safety pipped numerous times in last few days so I have no idea how to replicate it.
