How do you avoid DS in the endgame?

I have to agree. BT when it was based on terror radius makes sense for the 15 second. It has an obvious restriction to how it can be used and can be played around.
Now the only way to play around it is just wait 12 second, hope the person doesn't have BT, or go out of your way to find someone else, even if you weren't intentionally camping or tunneling. In EGC, you're just screwed if they have BT.
This all coming from a survivor main btw.

unless if the survivor is close to an exit gate, you can just out the timer that's what I do.

Yeah I especially hate how it works near exit gates. I feel like it should be nerfed that it automatically deactivates within a certain range of exit gates otherwise you get a free escape as well

Don't hook the survivor next to the exit, count to 12 before you hit them.

You don't. If the survivor gets off that hook, they're escaping guaranteed.

Now it's not even gonna be camping that gets you punished with BT. Now it's gonna be "Oops. I unintentionally stood 32 meters within the hook instead of 33. Guess they get a free health state." because most of the time they'll run at you with BT and DS, and you'll be the one punished for their stupid play.

BT will work across the map - The distance is irrelevant, now.
