I used to main Doctor so I know a fair amount of tips.
First things first is the power. Shock therapy has a static field so you can use that to deny stealth entirely as long as you check lockers. Tier one madness is actually a rather bad level to leave survivors in as it only offers a 33% to get a madness skillcheck and needs the highest grade of add-ons to actually have any afflictions.Tier two is better as they have a 66% to get madness skillchecks for the rest of the game, which can really slow things down. As a bonus, all of the uncommon add-ons generally work at this tier. At tier three survivors cannot do jack save for "Snap out of it" and hook saves. Being in this tier also denies item usage so the newer flashlight folk might waste time going for pallet blinds they can't get. It also causes periodic screaming.
His power is great for slowing down the game and it can be used to counter looping too. After being shocked survivors cannot drop pallets or vaults for a short time. So just aim ahead of them and charge in for THE KILL.
As for your perks, not much changes really. The only thing to be aware of is that Nurse's Calling reveals people snapping out of it.4 -

I used to run BBQ, distressing, brutal strength and ruin. I dislike skillcheck builds because good surivors are not affected by them, but if you are in low ranks, they work wonders.
Nurses calling can be quite helpful if you have problems tracking survivors, maybe switch it with distressing.
I prefer brutal strength over enduring because survivors tend to drop pallets earlier vs docAddons I recommend the discipline one that increases the madness rate and some range addon (even the common one is very good already)

Distressing because he is the only killer whos core power benefits from a bigger TR.
And at some point the survivor start ignoring the heartbeat and start healing anyway, because the TR is "everywhere" and NC helps you interrupt them and also snap out is shown with NC, which helps to keep them from doing it and make them stay longer in Madness 3.
He is one of the few killer that can slow the gens down without the need of using Ruin and risk of it being broken in the first 20 seconds of the game.0 -

I have him at P3 at this Point and the Bloody Cosmetics only make him Look even more terrifying. He is somewhat weak and needs buffs but I love him anyways.
But this how I would buff him:
- Make his Add-On Calm be part of his Kit
- Make him be able to instantly switch Modes
- Increase his Speed in Therapy Mode to 115
or 120%
- Increase The Tyme in which Survivors can’t
do any actions like Vaulting or Dropping a
Pallet
- Give him an Add-On that Inverts Survivors

Having played through most killers, sparky boi and mud gremlin mean the most to me. I love Mikey and I think with practice I could learn to enjoy the nurse, but I am definitely a Doctor main.0

I started as a doctor main, but I've moved on c_c Anyways, to add to what others have said:
- The doctors movement speed is slower when in treatment mode (shock therapy) than it is in punishment mode; it can make a big difference when trying to get somewhere quickly.
- His static field is as large as his terror radius, hence the mentioned terror radius perks/addons. There are times where his heartbeat can cover most of the map.
- Madness has a chance to make skill checks appear in random places on the screen, which sucks as a surv.
- Survivors cannot drop below madness level two by snapping-out-of-it.
- To snap-out-of-it they have to be standing still, and pass even more skill checks. If they fail them, they scream. Hence the skill check perks.
- Shock therapy hinders any survivors hit for 2.5 seconds.
- You can see each survivors level of madness by looking at their health icon; the stronger the presence of static is, the higher their madness level.
The builds already mentioned are a good starting point. If you don't mind hex perks, you might find it fun to try out huntress' lullaby. But, you know... hex perks.
My strat was pretty straight forward; hit the survs with shock therapy, then quickly pull out the ugly stick and thwap em with it. They generally expect you to do this, though, so sometimes they will drop a pallet prematurely to avoid being hindered, if they do, I found it faster to simply run around the obstacle than to break the pallet.

Even good survivor get trouble to get those skillchecks at some point.

Look up and play Sniper Build Doctor and you won't have to care about winning because the amount of BP you will farm can easily satisfy your lust for power. If the survivors don't plan gens out you can literally keep all 3 gens under-lock while standing still (I wouldn't recommend simply holding the game hostage but you can drastically slow the game down and get your hits).
I would say Doctor and Myers seems to have the most build variety in the game right now. Hag has gotten a few more to add to her kit too.
