Match Loading Even With 3 Survivors

Lobby timer went down to 0, even though there was only 3 survivors in the lobby. Then after the loading screen fully loaded, it produced an error code and then the match was cancelled. Why wasn't this detected in the lobby?
This has only happened once so far.
