Xbox - Soul Guard not working

Okay, so I started playing the new update yesterday and for some reason my soul guard was activated throughout the whole game, despite there being no hex totem, and/or being healed. When trying to recover from dying state to injured, it still told me to "Find Help" even though my perk was highlighted and was "active".
Edit: Unsure if this is this way for other consoles or PC
