Shoulder timing

I'm having some trouble getting the timing right on the instant shoulder action when a survivor is rescuing a hooked victim. It also seems that I just get a strike which seems to do nothing 50% of the time. When do I have to attack, and how if it's specific, to stop a hook rescue?
I know it's a total newb question but it seems really inconsistent so I thought I would ask.
