How does the ban system work, both on console and PC?

I'm confused a little bit about the ban system? people keep saying that if you DC like 90% of the game you'll get ban, or you'll get ban on some reports, like 3 unsportsmanlike like conduct reports then you'll get ban; because If the report part is true, how do they know the legitimacy of a report and aren't people just reporting because they become salty at a match they lost and stuff?
Because knowing the community, theirs a lot of toxicity in it...

    They have match data and many reports need additional video proof. 

    Also there are no console bans yet. They will be implemented "Soon tee em".
    Hopefully sooner than later.
