Second hook stage on first stage unhook

It happened me before I don't know if this is intentional or a bug.
On PC playing as survivor, I got hooked for the first time, I waited to be unhooked, but after a wile I tried to unhook myself. At my 3rd try I succeeded but my hook stage icon showed like I got hooked twice.
You can only try to unhook yourself at first stage, if I succeeded, that should mean I still got unhooked in first stage. But I get sacrificed if I get hooked again without ever entering second stage.
