On Sanctum of wrath, near a rock and tree placement, I was being chased (not looking where I was going), went up a rock near a tree, and fell in a hole.
The killer can get stuck too, and the only way out is for them to pick you up and wiggle out.
I’m on Xbox Series X, and this could occur multiple times if this isn’t patched.
