No clue what causes/caused this. it's the Killer charm, 'Pained Devotee':
Works like normal if I put it on the hook, though, I think.
(Ignore the original purpose of this post. I didn't realize Legion gets two Fifth Anni. crowns, one for Julie and one for Frank.)

I just noticed this, too, lol. I was putting some charms on my Oni hook and saw that one was blank but still works when clicked on.
I'm on PS4.
