Skill checks are not appearing

I was playing normaly, so I listened the skill check noise but the skill check doesn't appeared, in any circumstances (while reparing a gen, healing, even in the second stage of the hook). My game stay bugged until the match ends.
But this was the first time it happened to me, and I don't saw nothing like this, or anyone says about this bug. So I don't know how often this can occur, but it really can makes the match turn tô a disaster.
I play on PS4.
