Item loss after escape the trial

Play on PC
Step 1 : Bring the item and add-ons in the trial
Step 2 : Escaped through the exit gate with item
Step 3 : loss item and add-ons
****It was not shown item loss but it disappear that I can't see it in loadout
(the last game it happened, I also bring white ward)
ADDITIONAL INFORMATION
- Character played : Nea, Laurie etc. (Characters not specific)
- Perks played : Dead hard, Borrowed time, Decisive strike, Self-care (Perks not specific)
- Map : Suffocation pit is the last map the issue happened (Map not specific)
- Frequency of the issue : more than 5 times in 2 months
The problem is not big but it's so annoying when loss ultra rare item/add-ons
Please fix it
