Xbox - Adept Laurie Achievement won’t trigger

I played Laurie using only her 3 perks and no 4th perk. I escaped once by fully opening and leaving through the exit gate, and then escaped again a second time where I left through the hatch.
In both instances I was unable to get the achievement to pop.
I have double checked and I do not already have the achievement.
I also double checked that I was running her perks. This happened yesterday. The achievement still hasn’t popped for me.

- Perks played: DS, OoO, & SS
- Map: Coldwind and Autohaven
- Frequency of the issue: 2 out of 2
