Halloween Tome Challenge Bugged?

I’m doing the liberator challenge where I have to unhook 6 survivors safely, and I’ve played two games where i haven’t unhooked a single person. I was the one getting hooked first and being saved.

At the end of the game it somehow says I’ve unhooked 5/6. Is this challenge bugged or something?
