Make self care basekit
It's been 5.5 years. In a normal game I really doubt anybody can heal themselves more than 6 times. Unless they are the only person chased and get healed in between chases.
Self care now has 3 charges, once injured you can heal yourself in 16 seconds. If healing is interrupted it slowly goes down to 0%. (Like ruin)
After each successful selfcare heal you 1 charge gets removed and your next self care heal is 8 seconds longer.
While healing you gain new status, Selfish and can't be healed by others until you fully heal yourself with self care or wait for your progress to reach 0%.
Since it would inbuilt ability I wouldn't want to give it more penalties, but if I had to it would probably be something along the lines of. Status selfish lv2. "People can't trust your intentions, your actions are 5% slower until you work together with at least 1 other survivor for 45s." (Selfish lv2 would happen after 3 uses of selfcare)
Edit1: You can't use medkits if you have self-care. On yourself or others.

That's a little too convoluted, the devs do like simplicity in gameplay mechanics and having all that into a build in action without much explanation wouldn't really fit the vibe of the game.

Last I checked I had to actually bring it into the game through perk. CoH is pointless for me I would rather use medkit then run across the map to use it.

No, we don't need self-care actions to be base kit. Bring a perk (SC, CoH, Inner Strenght) or a medkit or find a teammates to get healed.
I think we would lose the fact we have perks to choose what playstyle we want.

I would much rather at least some of the meta perks became basekit and then adjust the rest. That way you could actually have new meta since old bandaid would be "healed"

How does bringing same 10 perks out of 100 allow you to choose your playstyle? I doubt that you run all non meta perks all the time to change up your playstyle.

I can imagine a world where every survivor ends up running to a corner of the map and self-caring like crazy at early levels, were this to be implemented.
Perks don't need to be added to base kit, but the idea of improving the HUD to give information to boost SoloQ up to swf is a far better idea. Information is more important, and part of the game excitement is the limited options for healing and safety. With COH and various other healing perks available, Self-Care should stay as a perk.

Doesn't matter since 9/10 games will have CoH on someone. Lets just not add a 5th perk for survivors ok?
