Is the Doctor killers ability supposed to be consistent?
I've just got done with playing like four rounds with the doctor killer, and I am absolutely sure that at no point when I trigger shock therapy and get a hit for the first time during a chase on a survivor did it ever increase their madness level. There were almost certainly more times where I charge and fire, it gets a hit indicated by a scream from the survivor, and then literally nothing happens. I've had a chase conclude with 5 consecutive hits with the shock therapy, and they end up at Madness 1, which is what they were at before the chase started, meaning absolutely no change.
Is this supposed to happen, where the ability only has a random chance to actually increase the level?
Is there a delay on the effect sometimes, where getting hit with a blast only increases the madness level after a brief wait period?
Is there a cooldown timer, where a survivor can only have their Madness level increased after a certain period of no change?
Is there just an unbelievable abundance of lag on my system, triggering hit actions when I never actually hit a survivor?
Reading the description for the ability on the killer, I can see that there is a distinction between "Madness" and "Madness Level", and that there seems to be no concrete way to guarantee an increase in Madness Level with abilities that only affect Madness, so what am i supposed to do? How else does madness increase? Am i supposed to shock them, chase them, causing their Madness to slowly rise until they trigger an increase in Madness level, only then shocking them again?
The inconsistency is extremely frustrating, and the lack of explanation makes it even more so. I could spend an hour surfing wikis until i understand what the doctor actually does, but I'd rather get an opinion from the community on whether this is genuinely supposed to be a thing, and if it works in a particular way, or if the doctor is just... not a good killer.

A shock therapy hit fills the madness meter by half a tier and a static blast hit does so by a full tier. That's how it always works, there's nothing in the game that changes that behavior.
Madness only increases from successful shock attacks of either kind, there is no other way.
There is no delay to the madness-increase.
There is no cooldown to madness, each successful hit should increase it, no matter what.
