Create your own Ultra Rare add on
Basic rules; if it is a significant increase in power, it must come with a significant drawback as well.
Examples:
Pyramid Head - Hitting a survivor with Punishment of the Damned will now apply Torment. Punishment of the Damned's range decreased by 1.5m
Nemesis - Increases the number of zombies by 2. Auras of zombies are no longer visible.

I wish Demogorgon had lopro chains for his M2 (for those who don't know, lopro chains are a Billy add-on that allows you to continue sprinting after destroying a pallet with your M2,) this would activate if you held down M1 while shredding.

I like it, but feel like it would leave very little counterplay to his power. Most of the pallet shreds happen because you either drop the pallet or take the hit, this would make it lose-lose. In Billy's case you lose 1 health state instead of 2 so it's beneficial to Billy but not as much as the pallet not being dropped in the first place.

Ghost Face
While Night Shroud is active, see the aura of any Survivor within 12 metres of any hook.
Ghost Face looses the ability to stalk.
Deathslinger
Your harpoongun can shoot 2 harpoons instead of 1.
The second harpoon has shorter reach. (about 12 metres)
You cannot reload until you fired all your harpoons.
The Oni
Start the Trial with your power gauge already full.
Downing a Survivor while in Bloody Fury instantly depletes 50% of your power gauge.

Nurse: while in post blink animation, you see auras of survivors within 6meters. However, post blink lunge is decreased by half
Blight: rushing on a dropped pallet will break the pallet but won't stop the rush. The number of maximum rush tokens is decreased by 2

Legion: After striking survivors 3 times with feral frenzy, the 4th will put the survivor into the dying state. This ends the frenzy.

Demogorgon: lose all his portals... the shred now become an insta-down ability.
Pig: lose all her traps... the ambush is now an insta-down ability.
Clown: yellow gas increase the player's speed at sprintburst speed... the speed bonus now last 3 seconds.

Trapper - Trap Toss. When holding a bear trap you have a little white circle that shows where you can throw a trap. The trap takes about 1 second to go through the air and set, and if it collides with a survivor they're instantly trapped. In the face. If the trap hits a wall or obstacle other than the survivor or ground, it is destroyed.
Wraith - Chaos Bell. Ringing the bell (cloaking or uncloaking) causes the survivor's screen to blur (think clown bottles) but worse the closer the Wraith is to the survivor. Survivor's camera will shift to face the Wraith. You can no longer cancel your cloak or uncloak.
Hillbilly - Unlimited Saw. You no longer have an overheat mechanic. Your chainsaw charge up time cannot be decreased but if ran with the add on that breaks pallets on hit you can still one hit down people.
Nurse - Really breathy breath. Breathing survivors reveal themselves with a static that increases when they're injured. (So the Nurse has a general idea of where you are.) Missing a blink hazes your screen for 10 seconds (So you can't just warp on people if they juke you. Like, extended exhaustion.)
Shape - Teleport. When no survivors are looking at you, you're able to vanish completely and reappear somewhere else on the map. You cannot teleport within 32 meters of a survivor. You cannot go into Tier 3. (I imagine this being used with Scratched Mirror)
Hag - Mud traps. Hag can no longer teleport to the survivor, but if they trip a trap they're slowed -a lot- and are pretty much sitting ducks. If multiple survivors walk in the mud trap they're slowed too until they exit the tile. Make it last a little bit.
Doctor - Moving Illusions. A survivor can now see illusions at Tier 1. At Tier 2 Doctors will start moving around the map with a terror radius. They're aggressive and can swing but cannot deal you a damage state. If they grab you on a generator you're instantly Tier 3 Madness.
Huntress - Bear Strength. Instead of Hatchets you have a wind up on your main attack. Fully charged downs people in one hit, but it takes like 5 seconds to do so. Run it with the add on that gives you 115% run speed.
Cannibal - Tantrum. Tantrum can be activated at will and you can move at normal speed during it. Uses all charges (so use it on your last charge). If you collide with an environmental piece at any point, you throw a tantrum, then another tantrum. You're exhausted after that and move at 110% for a minute and cannot use your chainsaw.
Nightmare - Face the Nightmare. You have the option of warping a survivor out of reality. They'll face a computer controlled Nightmare that chases them around the map with abilities a normal slasher can't have due to game limitations. For the survivor to survive they have to loop this superpowered Freddy and make it to a clearly marked exit area. If they get caught, they get a special mori. Welcome to prime time.
Pig - Window Traps. You think ahead. You have 10 Window Traps instead of Bear Traps, which follow the same rules as the Bear Traps. You can place them on pallets and vaults. When active and the killer isn't chasing them, the survivor is slowed severely while the clock ticks down. Great. You escaped the chase. I want to play a game. You did it to yourself.
Clown - Pink Elephant. Clown can now bull rush the survivors. Running into a wall honks your nose and stuns you for a bit. Can technically be used for travel but if the Clown catches you then you're instantly picked up without losing a health state. Hope you can wiggle off champ.
Spirit - Walking Phase. Your husk now walks around and chases nearby survivors but can do no damage. You also can no longer see your husk while phasing. Upon reappearing the husk explodes and makes nearby survivors scream.
Legion - Team up. Other Legion members walk around with the same AI as Nemesis zombies, except a little faster and can injure. You cannot see them on the map or what they're doing and they can be knocked unconscious by a pallet drop. If one goes down the final legion member spawns. If they're all 3 defeated then they stay down.
Plague - Full throttle. You now have brown puke from the other side. Causes intense sickness to any survivor unfortunate enough to be in the spray. A clever Plague may be able to aim without being able to see, but unlikely. Probably decrease the amount of liquid that comes from the front to compensate.
Ghostface - Buddy up. There are now two ghost faces on the map. One AI controlled. You do not get credit for his kills and Ghostface will attack you and sabotage your chases. You're stunned longer and may trip. You may kill your buddy or he may fall to pallet slams, but it's going to be chaos. A high chance of falling down stairs in a goofy manner. Like the movies.
Demogorgon. Abyssal Map - Your teleports start placed next to all the generators. You don't get them back if they're cleansed, but they take a very long time to cleanse and you have killer instinct regardless of if you're channeling the abyss.
Oni - You're now in the blood fury state all match, but you lose the ability to M1 survivors unless you're carrying one. Your kanabo must be fully charged before hammering it down. Blood covers your screen for 30 seconds on successful down of a survivor.
Deathslinger - Explosive gun. Your gun can malfunction and explode on command. You lose the ability to use your gun for 2 minutes but any survivors impaled while the explosion happens are instantly downed. If used while not being fired then you're stunned for 10 seconds and all survivors within 12 meters of you are downed.
Executioner - Nurses (no not Sally) Silent Hill Nurses join the fray and act like Nemesis zombies. If they are slain they cannot be revived.
Blight - Splash of Serum - If you run into a survivor without hitting them after the first dash, you splash orange serum on them. This cannot be removed for the rest of the match and leaves an orange trail when they run (not walk) and it glows slightly. It takes them slightly longer to heal due to the toxin.
Twins - Active other half - While you're in control of Victor, Charlotte can move around independently and attack survivors. Double the transition time and realize Charlotte may not be in a tactical place when you do come back. Victor does his own thing when you're in control of Charlotte but he's turbo aggro and not careful.
Trickster - Getting out of lockers lacerates a survivor with knives for one health state. Knives can now be stuck in generators and exit gate panels for 1 minute, damaging the survivor every 5 seconds he interacts with the object. (This could be entirely useless if you're not around to finish the job.)
Nemesis - Rocket Launcher. You now have three missiles you can fire in exchange for not being able to use your infection whip. The rockets instantly down all survivors in the area that are hit. Hope you're a good shot, otherwise you're a fist wizard after you mess up. It is very, very obvious you are running this add on.
Cenobite - True puzzle. No box spawns in the trial, but after a survivor is hooked a piece of the box falls down. The pieces fuse if brought together. If six pieces fuse then the puzzle box becomes whole. If a survivor activates the box then the Leviathan chains you for the rest of the match, effectively banishing you. If you stop the survivor from completing the banishment you can instantly Mori him and the box is removed from the match. You lose your passive pressure in exchange for high risk Rancor.
Artist - Flock You no longer control your birds. Flocks and raven heads fire off occasionally without input from you, but generally heckle survivors. You no longer see crow outlines to tell you where survivors are.

Huntress: Whenever you gain the Undetectable status effect, your Lullaby radius covers the entire map. You gain the Undetectable status effect when reloading for 30 seconds.
(Replaces both the Soldier's Putee and the Wooden Fox; the former becomes a brown rarity addon and the latter is reworked entirely.)

- The Auras of all Survivors not caught in a Reverse Bear Trap are revealed to you while a Survivor is searching a Jigsaw Box.

Trapper - You can grab traps from lockers. If you do the oldest trap on the map is destroyed. You can not see the auras of placed traps.
Wraith - Hitting a survivor automatically recloaks you at double cloaking speed.
Hillbilly - Your chain rev speed increases 100%. Once you start revving you can not cancel the rev.
Nurse - You move 115% speed. You can not attack after a blink.
Shape - During Evil 3 your speed breaking pallets, walls, damaging gens, vaulting, and recovering after a basic attack hit is increased 100%. You can not instant down survivors.
Hag - Your traps act as AI controlled zombies that chase survivors for 5 seconds before vanishing. You no longer teleport to traps.
Doctor - Survivors in Madness 3 have their movement controls reversed. Your Shock range is reduce 2 meters.
Huntress - Your hatchet charging speed is increased 100% and you move normal speed while charged. You can not make basic attacks.
Cannibal - Your chainsaw rev speed is increased 100%. You can not make basic attacks.
Nightmare - You are Undetectable and cloaked to awake survivors. Hitting awake survivors always puts them to sleep but does no damage.
Pig - Traps automatically arm when placed, you have unlimited traps, and the timer on traps decreases 10 seconds per trap placed. You can not hook survivors.
Clown - You move 120% speed. You can not throw pink bottles.
Spirit - Your phase recharges 100% faster and you can use basic attacks while phased (dropping you out of phase automatically). You automatically phase whenever it is charged
Legion - You look like one of the survivors and are Undetectable when not in Feral Frenzy. Your base movement speed is 110%.
Plague - Your basic attacks instantly fully infect survivors. You can not infect survivors at range.
Ghostface - You are always Undetectable. You can not stalk.
Demogorgon. - When holding the Power Button you detect the killer instinct of the closest survivor to you. You do not detect instincts near tunnels.
Oni - Your basic attack hits charge your ability 100% faster but do no damage
Deathslinger - Your movement speed is 115%. You must recharge your rifle at lockers
Executioner - Tormented survivors are Exposed. Your ranged attacked inflicts Torment but does no damage.
Blight - Your Rush tokens recharge 100% faster and your turn speed during rush is increased 30%. Your base movement speed is 110%.
Twins - The speed of switching between the Twins increases 100% and Victor recharges 100% faster when destroyed. Charlotte’s base movement speed is 110%.
Trickster - Your daggers ricochet 3 additional times. They only inflict damage after at least one ricochet
Nemesis - You have 3 additional zombies. You only see the aura of the closest zombie to you.
Cenobite - Survivors who open the box are Exposed for 60 seconds. Chain Hunt takes 15 seconds longer to activate
Artist - Your Dire Crows break pallets and breakable walls. Hitting a dropped pallet or breakable walls stops their flight.

Fling Traps: You gain the ability to throw traps at survivors heads as if they were heat seeking missiles.
Drawback: You can only carry one at a time and must reload at lockers.

Clown - The Afterpiece tonic causes the survivor to see pink elephants (Like the Mr Puddles charm) randomly float about on their screen, also causes the oblivious status and intermittent elephant trumpeting noises for 4 seconds, removes other abilities.

Legion: have it to where the 2nd hit of their power able to down the player and hitting a healthy survivor leading to them only getting half the time to mend before going down to the deep wound
Cons upon downing someone with it stops the power,the auras of others survivors aren't shown when you land a hit with the power
Nemesis: changes the ability to a one use rocket launcher that's capable of downing healthy survivors, knocking 75% of a gens progress and destroying undropped pallets if hit by the rocket
Cons:you have no zombies and can't use your special anymore once used and are 5% slower

Spirit- ( Ultra Rare add-on ) :
( Rin’s Revenge) :
The sight of your dismembered limbs fills you with even more hatred and resentment toward your father.
( there will be a dismembered limb icon to match the add-on details. )
Pros:
-Immediately after exiting phase-wall, The Spirit lets out an angry yell ( like the one in her Mori ) and has a window of 0.5 seconds or a tiny bit more to instantly incapacitate healthy survivors.
-Allows the Spirit to see the shadows ( NOT AURAS ) of survivors within lunge distance.
Cons:
-The Spirit can be SEEN charging up her power while she loses sight of survivors as usual ( the hand twitching that happens from the Spirit’s POV while charging up her power to go into phase-walk ) and her HUSK no longer has collision ( meaning survivors can run/go through it just like how she’s able to.
OR
Mother-daughter ring - ( current icon and name ) :
Pros:
-The Spirit can go through any wall/object ( all types of stairs aren’t affected by this meaning she’ll NOT clip through them and instead go up the stairs ) while phase walking except for the debris around certain tiles ( like the ones in Autohaven wreckers, for example, the cars and whatnot and this applies across all maps that have this ) as these loops tend to be really unsafe against fast-moving killers.
Cons:
-Decreases movement speed to 136% ( -40% ) and cannot be further stacked with other movement speed-increasing add-ons.
-The Spirit no longer “ passive “ phases.
I don’t really know how these would fare if they ever were implemented. They might be bad or not but it’s fun thinking up add-ons.

When I made my first Freddy concept, I wanted his Ultra Rare add-ons to be a least a bit similar to his current power, so that new Freddy mains would not go through what I had to endure.
I eventually abandoned the idea for RPB (not for Black Box tho), but here is the remnant of that concept:
Red Paint Brush:
-Tremendously decreases The Nightmare's Terror Radius.
-Tremendously reduces Dream Transition time.
Time to fall asleep is reduced by 2 seconds.
-Survivors suffer from a Hindered penalty during the Dream Transition

A technically advanced version of the Reverse Bear Trap. It shares many similarities with the original but is more stable and consistent.

Reverse Bear Traps placed on Survivors are active immediately.

Blades are recovered with the time.
Blades go throught objects one time producing half of fill meter instead of full.
Spirit
Original Katana: Spirit and survivors can see a glympse of each other while on phase.
Nulify all sounds from spirit.
Pyramid head:
Hitting someone with punishment of the dammed when tormented becomes insta downed.

Deathslinger - Bayshore's Lighter
A longtime keepsake of the traitorous Bayshore, Caleb pocketed it before leaving him and the Warden for dead at the hands of the Hellshire Gang. Although the lighter has long since produced it's final spark, it still stirs something fiery and all-consuming inside of him.
Speared attacks inflict Incapacitated on the survivor for 30 seconds. Survivors that break free from a spear are hindered by 7% for 30 seconds. When you aim the Redeemer, let out a angry roar that causes all Survivors within 24m of you to scream and reveal their locations. This has a cooldown of 45 seconds.

Hag - You can teleport to trigged Mud Phatasms. This add-on trumps all other add-ons.
So basically just something to use the other Iri/purple add-ons with. Drawback is that you essentially only have 1 add-on, since this one has no other purpose than making the teleportation possible.
