Survivor: P3 Ash, Complete Water Trash cosmetic.
Hooked in the basement, first hook.
Jumped off the hook with my last attempt but was classed as second hook state after successfully unhooking myself so died on my next hook.
My perks: Resilience, Spine chill, Iron Will, Dead Hard. all Tier 3. Flan offering.

Yeap, happens to me too. Unhooked at last attempt and died on next hook. TLDR : if you jumped off the hook on last attempt= it consume your hook state.
