Dead hard animation starts but doesn't work?

has anyone had issues with dead hard not registering ?
several times now I've pressed it within plenty of time before getting hit (within the dead hard timing) and it hasn't worked.
it's kind of infuriating when it has to be perfectly timed anyway but then when it doesn't work when you've blatantly pressed it at the right time .....

My first thought is that endurance effects do not stack. Even though this is old news, I still see survivors hit with BT/OTR trying to dead hard the next M1.
