We have temporarily disabled Firecrackers and the Flashbang Perk due to a bug which could cause the Killer's game to crash. These will be re-enabled in an upcoming patch when the issue is resolved.

The Outbreak breakout achievement simply doesn't add new escapes.
I have escaped through the gates 5 times after single-handedly opening the gate from 0% to 100%
Still only have this one pip I got on Project W's launch.
