Is there a reason Bite The Bullet doesn't stifle your sounds when being healed ?
I've been messing around with Bite The Bullet for a few days, and it's been a bit frustrating to notice that :
When healing someone, both your sounds are stifled.
When healing yourself, your sounds are stifled.
When being healed, you're still loud as all hell !
It does not feel very coherent to me. Is there a reason it works that way ?

Because the perk only activates when the person with the perk is engaging in the healing action.

I guess that makes sense, though the name is not very coherent with it then..

This is probably the part that bothers me most.
The flavor text says, "You don't like it when other people see you in pain," and it doesn't silence your grunts at all if you're being healed by someone else.
The perk isn't even exactly OP or anything. It can be useful and funny in certain scenarios with things like Lucky Break and green medkits, but that's a lot of your build dedicated to something that can be defeated by object permanence.
