Self unhook last sec straight into death hook

After I freed myself from the first hook in the last second, I died directly the next time the killer hooked me. UI and everything showed me on my first hook. If it had been a pure display error, I would not have been able to jump off my hook in the first place.
Killer also saw me on the first hook stage. No perks or offerings regarding self unhook or luck were used.
Unfortunately, I have no video of it.
