    I don't think it's the same since Bill died after sacrificing himself. The entity probably took his soul.

    Yh diseases probably don't work the same in the entities realm. Survs don't retain diseases inflicted in the trials, like Plague's well.. plague. Nemesis's T virus, Wesker's orouboros. Even insanity induced by the doctor clears up. So either the Entity's realm controls how diseases work and only allows them to affect survs in the trials. Or all of the infected survivors die soon after the trial and are resurected free of the disease. Or perhaps they just find a vaccine, spray or fountain of devotion somewhere in the realm. And perhaps the odd therapist sitting around to help clear up the insanity lol

    Actually a pretty interesting question. Considering Bill was dying/has died when the Entity took him, he probably got reincarnated in some way (via the Entity giving him a new body), so he most likely wouldn't still be carrying the Green Flu.

    Based on the description of Auric Cells and some of the Observer's lore, the Entity's Realm isn't a physical place per se but a place of pure consciousness.

    Think of like a lucid dream, but the Entity is the lucid dreamer and everyone is else is trapped there, beholden to the Entity's rules.

    Bill's body isn't his real body, but a body based off his and recreated by the Entity so that his consciousness can feel suffering, hope, and despair.

    Whether this body is infected with the Green Flu or not is irrelevant because the Entity has near absolute control. It's how Survivors die and return after trials, why they turn into a smoky mist shape after being sacrificed, and how people who physically died can still be present in the Entity's Realm.

    Nobody else will be infected if the Entity doesn't want them infected.
