Step 1 : Boot the game
Step 2 : Play as killer (The Twins) in a public match
Step 3 : In-game, change into Victor and look on Charlotte OR perform a mori on a survivor.
Step 4 : Notice that the Charlotte's head piece is detached and flying above character. The same with the accesories on the back.

- Frequency of the issue - every match
It's happening also with Blood & Guts skin set as I'm aware.
