Error code 500 , my game keeps crashing

For about three e weeks now since the middle of December every time I load into a match 1-3 minutes in and I get randomly disconnected. I’ve even got a new wifi router and still did t help I play on XBOX ONE S idk if it’s my xbox or what but please help
