Decisive strike not activating mutliple games now

Getting unhooked first time, decisive strike wont activate, unhooked second time, decisive strike wont activate.
Been happening a few games in a row now, no I did not get picked up and dropped again and no the gates weren't powered.
Also there has been a rainbow map bug if you fully use your map it still works untill you let go of M2.
