Tome 9 Challenge Not Working

I have tried to complete the Tome 9 Level 2 challenge, OUT ISN'T AN OPTION. I have completed games where I have killed every survivor in the match, and have one escape through the hatch without having any of the two exit gates open and still haven't gotten the challenge completed. I have tried many times but fail to finish with no apparent reason.
