Dead hard should activate after unhooked.
Don't punish killers who don't tunnel so don't give it to unhooker. Why would killer go after someone with 3 health state instead just someone with one. Dh would be with this change max 2 time use per survivor and killers would not have to deal with it in first chase when it's most punishing. Also after using dh survivor should not be able to drop pallet or vault a window for few seconds as well.

You wont deal with it in the first chase, since someone needs to unhook another survivor in the first place for it to activate

its just a worse otr at that point

Killer can immediatelly hit OTR away but you can instead use Ds to take away the deep wound and then Dh which will not have timer so you can use it later if killer does not tunnel.

It still works later if you don't get tunneled and I think Ds and Dh combo is better counter for tunneling than Otr.

It is pretty common situation to go back to hook and choose either unhooked or unhooker to chase and the latter choise is made not worthy.
