Is Pain Res bugged or am I not understanding the rework?

Pain res used to be one of my comfort perks and it was fairly balanced. Now with the rework it only has 4 stacks, however do stacks share between the 4 hooks or is it 1 stack per hook? Also in the spanish translation of the perk it says that it activates with the first hook of every surv, dont know if thats true to the english version.

I ask this because I played some games with the new version and sometimes the perk doesnt consume a token, other times it does even if the surv is on second hook stage and hooked on a previously used scourge hook. Im really confused as to how this perk works now. Hope someone can clarify it to me ^^
