When a killer hooks the same person twice, they get the same value even if they haven't hooked one survivor in specific.
In the description it's very clear that it has 4 different tokens for each individual survivor, but it's not how it's working in-game. They can very much hook the same survivor twice and the gen will explode twice, however, the token is still consumed on use.
It's still not intended and it's so bad for the game because it makes you make decisions that won't work due to the gen exploding anyways, even if you know the person was hooked twice.
