The first one is their Seabed Sustenance torso cosmetic. The body does not align correctly with any of her cosmetic head options, including the head that comes with the outfit, allowing you to see into the model.
The second bug has been in the game for a while.
The veil from both Eternal Anguish and Eternal Sorrow slip into where the head and torso fuse together during the main menu animation, causing the veil to clip into the front of Charlotte body, essentially creating a bib. This also appears during the endgame tally screen.
