How does Hex: Pentimento work?
How do I counter it as a survivor? It can't just be that we get debuffed and can't do anything about it, can it?

Cleanse a totem (hex or not) a killer can rebuild that totem and they gain a stack of pentimento but can only be rebuilt once.
Each stack gives a certain debuff, make sure not to give the killer 5 stacks.
If you cleanse a totem and you see the hex pentimento notification, recheck that totem to see if it was rebuilt. If it was re-cleanse it and you deny the killer 5 stacks.

You can’t counter it anymore if the killer gets all 5 so make sure that doesn’t happen. Otherwise you just cleanse the totem again and then that totem is permanently gone, the same totem can’t be rekindled more than once.
Pentimento totems can also be distinguished from regular hex totems. They look similar to the regular ones, but also have an additional red-orange flame around the totem (it’s a deeper red-orange than the normal hex totem color). You also won’t have the option to bless them, they need to be destroyed.
Having said all that, sometimes the best counter is to not give it to the killer in the first place. Plaything/Pentimento is a somewhat popular combo, but depending on the map and the killer you’re facing, it may be viable to simply leave Plaything up and avoid Pentimento that way.

To expand on what the previous two said, here are the effects per-active pentimento totem
- 1 Totem: reduces all Survivors' Generator Repair speed by 20/25/30 %.
- 2 Totems: reduces all Survivors' Healing speed by 20/25/30 %.
- 3 Totems: reduces all Survivors' Recovery speed from being downed by 20/25/30 %.
- 4 Totems: reduces all Survivors' Exit Gate Opening speed by 20/25/30 %.
- 5 Totems: all Totems are permanently blocked by The Entity
