instadeath on first hook, is this a bug or hacking?

Hi all,

Another survivor and I just died on the first hook state. I was the first to be hooked and the entity immediately spawned and took me. It was as if we had both previously been hooked twice before, but both of us had no previous hooks. The other survivors were still running around doing objectives. What is happening?

I just had this happen while playing solo survivor against Skull Merchant on RPD wing that includes the balcony. Just wanted to know if this is a bug or hacking.

    From what it sounds like, probably hacking

    Could be either really. If you run into the same killer again and it happens again report them.
