What are Bleeding points awarded for exactly?

Occasionally as killer, I'll get a points notification in the Deviousness category for Bleeding. I have Bleeding Survivors in every match, all killers do. So what exactly is it that triggers the game to only Occasionally give me these points?

Bleeding points are time based for as long as you bled a survivor out on the ground or are injured before they are healed.
A survivor bleeding on the floor for 4 minutes is worth 250 bleeding points, which is basically meaningless.
