PS4 - Dedicated Server disconnected

Was in a match against Sadako. I noticed early on there was random red patch on the ground. Later on Sadako started to nonstop teleport and I was disconnected with error message "Dedicated Server is not responding disconnecting." I received a 5 minute penalty even though it was a Dedicated Server error not my internet (no icons popped up indicating issues during match and I even tested my internet after to make sure it was good just in case). According to the end game screen I was the only one on PS4
Steps to recreate:
1. Public match against Sadako, crossplay on, be the only one on PS4.
2. Have Sadako teleport back to back over and over until Dedicated Server disconnects you.
