PC - "Relic Veneration" ritual did not get completed

As can be seen in the screenshots, I escaped a match after finding a Broken Key in a chest, yet my Relic Veneration ritual did not get completed. I also had an EMP in hand as I escaped; could this be what is causing the issue?
