During my game I was using Self-Care for the survivor master challenge "WORKING UNDER PRESSURE". Self-Care did heal me, but the bar showed no progress, I also tried failing skill checks to hurt myself with it would fix the meter, it did not break my progress nor did it do a fail skillcheck animation. The perk at least still works but it doesnt show the bar filling up like the medkit I used.
