Steps to reproduce: Unsure, but might be related to losing connection/disconnecting while dying on hook.
Just had a game where the 3rd person got hooked and died on hook. Then 15 seconds later they got disconnected whilst dead and got replaced by a bot, only then the hatch spawned.
